Recent advances in the treatment of inflammatory bowel disease (IBD) include new medical, surgical, and endoscopic therapies. Mesenchymal stem cell therapy is still in its infancy, although several studies suggest that the immunomodulatory effect of stem cell therapy may reduce inflammation and tissue injury in patients with IBD. This paper discusses randomized clinical trials and their potential role in intralesional transplantation of mesenchymal stem cells. Autologous stem cell therapy may be considered a safe and effective therapeutic approach for healing perianal fistulas in Crohn's disease patients. We conducted a comprehensive literature search to identify studies examining the role of stem cell therapy in inflammatory bowel disease (IBD). These studies raise safety concerns regarding the systematic application of mesenchymal stem cells, since several clinical trials have reported worsening of inflammatory bowel disease following intravenous infusion of mesenchymal stem cells.
